Summary

The Turkmen text reads, "Turkmenistan Ministry of Health Service and Pharmaceutical Industry" "Ashgabat City Health Service Directorate" "Ashgabat City 4th Health House Bagyr Branch" (i.e., the Bagyr branch of Ashgabat City Health Clinic No. 4, in proper English).
