May I suggest to add the area(s) of expertise of the practitioner:
- general_medicine=yes|no
- internal_medicine=yes|no
...
- dermatologist=yes|no
- gynaecologist=yes|no
N.B. amenity=dentist still seems to be a separate tag

At the time of writing, taginfo had the following statistics:

How to "tag the size"
The page states:
"please tag the size of the office in some way"
but offers no suggestions of how to do this. None of the listed tags indicate size, there are no links to relevant pages, and searching the wiki for "size" doesn't return any obviously relevant pages..
If the page doesn't have advice on how to indicate size then this phrase should be removed.
- You are right, I don't think there is any established tagging for the size, and it's not even clear what "size" would mean for a doctor's office (number of staff, number of phyisican staff, number of patients, number of rooms, number of treatments offered...?).
